How to File a Complaint

The National Consumer Complaint Database (NCCDB) is a resource for consumers, commercial motor vehicle (CMV) drivers, and industry professionals to report safety violations related to the CMV industry. If you’ve experienced problems with a moving company, truck or bus company, broker, auto transporter, or other entities within the CMV industry, you can submit a complaint through this system.  

Follow the steps below to file a complaint. Once you’ve submitted your complaint, FMCSA will notify you if the complaint is actionable or non-actionable once your complaint has been reviewed.  

IMPORTANT: If this is an emergency, please call 911 immediately.  


Step 1: Go to the National Consumer Complaint Database Homepage at nccdb.fmcsa.dot.gov.

Step 2: Select the Filer Category 

  • Consumer: 

    • Used interstate movers, brokers, or auto haulers  
    • Ridden a bus, limousine, or motor coach as a passenger  
    • Witnessed a safety problem on the road  
    • Received shipment of hazardous materials  
  • Driver: 
  • Industry Professional: 
    • Bus and truck companies  
    • Shippers  
    • Insurers  
    • Brokers  
    • Owner-operators  
    • Consortia/third-party administrators

Step 3: Select Who the Complaint is Against 

  • Moving Company
  • Medical Review Officer  
  • Property Broker  
  • Electronic Logging Device Provider  
  • Substance Abuse Professional  
  • Bus Company  
  • Truck Company

Step 4: Follow the Prompts and Provide Your Incident Details. 

You’ll be asked to:  

  • Fill out details of the complaint: Be specific — include dates, times, locations, and what happened.  
  • Answer questions: Provide additional information.

Step 5: Select the allegation category and subcategories (e.g., truck safety, hazardous materials, operating authority, etc.).

Pick the option that best aligns with your complaint. Depending on the allegation selected, a subcategory selection may be necessary.

Step 6: Enter Your Contact Information 

Enter your first name, last name, and phone number which is the minimum required information to submit a complaint. Please note that if you would like to receive a notification by email whether your complaint is actionable or non-actionable, you must provide an email address. If you do not provide an email, you will receive your notification letter by mail.  

If you are filing on behalf of someone else, please check the box next to the statement at the top of the page. Finally, select whether you would like to share the allegation only or the allegation and contact information with reported company.

Step 7: Search for Company Information

Select the company you're filing the complaint against by searching for the company name or the DOT number. If you're unable to find the company, you can enter the company’s information manually.

Step 8: Provide Documentation

If you have photos, documents, videos, or any other files you would like to share as part of your complaint, you may upload them during this step. Acceptable file types include DOC, DOCX, XLS, XLSX, GIF, JPEG, JPG, PDF, TIF, TIFF, TXT, WPD, ZIP, MSG, MP4, RTF, PNG, MPG, WMV and MOV. The size limit for the file is 2 GB. 

Make sure you add a description of your documentation in the text box before you upload. If you do not have documentation to upload, type "N/A" into the text box.

Step 9: Review & Submit 

  • Check that the information you entered is correct. 
  • Scroll to bottom of page to certify statement and submit complaint. 
  • If you would like a copy of your complaint, ensure you print or save your complaint page prior to selecting the "Submit Complaint" button.

Step 10: Save Your Complaint and Case Number

Once you've submitted your complaint, a window will pop up with your case number. Please save your case number.

FMCSA Will Notify You

After FMCSA receives your complaint, an email indicating whether the complaint is actionable or non-actionable will be sent to the email address you provided.
