Identity Verification Enrollment Centers
BEFORE visiting an enrollment center, please contact the FMCSA Registration Contact Center at 1-800-832-5660 for assistance/questions about the identity verification process. For more information, such as business hours, parking, and additional details about the enrollment centers listed below, contact the IDEMIA Enrollment Center Hotline at 1-833-832-5530. All enrollment centers operate on a walk-in basis.
If you are starting the FMCSA Identity Verification process, you must already have applied for a USDOT number via the Unified Registration System (URS) and received an Application ID. If you were unable to scan the QR code provided or couldn’t complete identity verification, proceed to the next step.
Step 1: Contact FMCSA for Assistance
Have ready: Login.gov email, your Application ID from URS and any error codes you received. You can:

📌 Note: You can submit tickets any time, any day. FMCSA will respond within 2 business days.
Step 2: Follow FMCSA Guidance
Wait for and follow instructions provided by FMCSA Customer Service. If instructed by our agents to go in person to one of the centers listed below, take into account that they can provide you with limited assistance: scanning the QR code, scanning your credentials, and/or scanning your face. All enrollment centers operate on a walk-in basis.
Step 3: Find a Location
To locate an enrollment center, you can perform a search by entering (part or all of) the name or city of the center and/or filter all locations by state. Additionally, you can perform a proximity search by entering a location and a distance (in miles) from the location to identify the closest locations.
