This collection of information is mandatory and is required by 49 CFR Part 385 and authorized by 49 U.S.C. 504

(1982 & Supp. III 1985). The Form MCS-150, Motor Carrier Identification Report, must be filed by all motor carriers

operating in interstate or foreign commerce. A new motor carrier must file Form MCS-150 before beginning

operations, Exception: A motor carrier that has received notification of a safety rating from the Federal Motor Carrier

Safety Administration (FMCSA) need not file the report. If you are a Hazardous materials shipper, but not a motor

carrier, you are not required to file this report. This information will be used to identify motor carriers subject to the

Federal Motor Carrier Safety and Hazardous Materials Regulations. Carriers may voluntarily update information

using this report.

The public reporting burden for this collection of information on the Form MCS-150 is estimated by the FMCSA to

average 20 minutes per response, including the time for reviewing instructions, searching existing data sources,

gathering and maintaining the data needed, and completing and reviewing the collection of information. Please

note that an agency may not conduct or sponsor, and a person is not required to respond to a collection of

Enter the legal name of the business entity (i.e., corporation, partnership, or individual) that owns/controls the motor carrier/shipper

operation.

If the business entity is operating under a name other than that in Block 1, (i.e., "trade name") enter that name. Otherwise, leave blank.

Enter the principal place of business street address where safety records are kept.

Enter mailing address if different from the physical address, otherwise leave blank. Also, applies to #8, #9, #10 &13.

Enter the city where the principal place of business is located.

Enter the two-letter postal abbreviation for the State or the name of the Canadian Province/Territory or Mexican State corresponding with

the physical address.

Enter the zip code + 4 number corresponding with the physical street address.

Enter the city corresponding with the mailing address.

Enter the two-letter postal abbreviation for the State or the name of the Canadian Province/Territory or Mexican State corresponding with

Motor coach—a vehicle designed for long distance transportation of passengers, usually equipped with storage racks above the seats

and a baggage hold beneath the passenger compartment.

School Bus—a vehicle designed and/or equipped mainly to carry primary and secondary students to and from school, usually built on a

medium or large truck chassis.

Mini-bus/Van—a multi-purpose passenger vehicle with a capacity of 10-24 people, typically built on a small truck chassis.

Limousine—a passenger vehicle usually built on a lengthened automobile chassis.

Enter the number of interstate/intrastate drivers used on an average work day. Part-time, casual, term leased, trip leased and company

drivers are to be included. Also, enter the total number of drivers (Interstate/Intrastate) and the total number of drivers who have a

Commercial Drivers License (CDL).

Interstate—driver transports people or property across State lines, including international boundaries, or wholly within one State as part

of a through movement that originates or terminates in another State or country.

Intrastate—driver transports people or property wholly within one State.

100-mile radius driver—driver operates only within a 100 air-mile radius of the normal work reporting location.

Anote la dirección física del lugar principal de la empresa donde se mantengan los archivos de seguridad.

Anote la dirección postal si es diferente de la dirección física, de no ser así, déjelo en blanco. Lo mismo se aplica a los números 8,

Si es un autotransportista o cargador mexicano, anote la colonia o barrio mexicana donde se localice el lugar principal de la

empresa.

Anote el número de teléfono, incluyendo el código de área, del lugar principal de la empresa.

Si es un autotransportista o cargador mexicano, anote la colonia o barrio mexicana que corresponda con la dirección postal.

Anote el número de fax, incluyendo el código de área, del lugar principal de la empresa.

Anote el número de identificación asignado a su operación de autotransportista por parte del Departamento de Transportes de los

EE.UU., si lo conoce.

Anote el número de autotransportista "MC" o "MX" bajo el cual la Administración Federal de Seguridad de Autotransportes (FMCSA) o

la Comisión Interestatal de Comercio (ICC) expidió su autorización operativa apropiada.

Anote su número de Dun & Bradstreet de empresa (que se usa como identificador secundario; si usted no tiene uno, déjelo en

blanco).

Anote el número de identificación del patrón (EIN #) o el número de seguro social (SSN #) asignado a su operación de

autotransportista por el Servicio Interno de Recaudación.

Anote su dirección de correo electrónico de internet, si tiene una.

Marque con un círculo el tipo de operación de transportista.

A. Interestatal. B. Intraestatal, transportando materiales peligrosos (49 CFR 100-180).

C. Intraestatal, SIN transportar materiales peligrosos.

Interestatal--transportación de personas o bienes al cruzar los límites estatales, incluyendo las fronteras internacionales o

enteramente dentro de un estado como parte de un translado de un lado al otro que origina o termina en otro estado o país.

Intraestatal--transportación de personas o bienes enteramente dentro de un estado.

Si usted es tanto un autotransportista como un cargador de materiales peligrosos, marque con un círculo el tipo de operación de

cargador.

Fletero Autorizado — transportación por compensación como un transportista común o contratado de bienes, propiedad de otros, o

de pasajeros bajo las disposiciones de la FMCSA.

Fletero Exento — transportación por compensación de bienes o pasajeros exentos de la regulación económica de la FMCSA.

Privado (Bienes) — significa una persona quien provee la transportación de bienes por un vehículo motor comercial y no es un

autotransportista fletero.

Anote el número total de los vehículos de su propiedad, arrendados por período y arrendados por viaje que son o pueden ser

operacionales el día en que este formulario se llene.

Los vehículos de pasajeros se definen como:

Autocar — un vehículo diseñado para la transportación de pasajeros de larga distancia, normalmente equipado con estantes arriba

de los asientos y un portaequipaje debajo del compartimiento de los pasajeros.

Autobús Escolar — un vehículo diseñado y/o equipado principalmente para llevar estudiantes de primaria y secundaria hacia y

desde la escuela, normalmente construído en un chasís mediano o grande de camión.

Microbús\Van — un vehículo de pasajeros multiuso con una capacidad de 10-24 personas, típicamente construído en un chasís

pequeño de camión.

Limusina — un vehículo de pasajeros normalmente construído en un chasís alargado de automóbil.

Anote el número de conductores interestatales/intraestatales que emplee en un día de trabajo promedio. Debe incluir conductores

de tiempo parcial, casuales, arrendados por período, arrendados por viaje y de la compañía. También, anote el número total de

conductores y el número total de conductores quienes tengan una Licencia de Conductor Comercial (CDL).

Interestatal — el conductor transporta personas o bienes cruzando los límites estatales, incluyendo las fronteras internacionales o

enteramente dentro de un estado como parte de un translado de un lado al otro que origina o termina en otro estado o país.

Intraestatal — el conductor transporta personas o bienes enteramente dentro de un estado.

Conductor de un radio de 100 millas — el conductor opera sólo dentro de un radio de 100 millas aéreas del lugar donde

normalmente se presenta a trabajar.

En el espacio provisto escriba con letra de molde o a máquina el nombre del individuo autorizado para firmar documentos a nombre

de la entidad indicada en el Bloque 1. Este individuo tendrá que firmar, fechar y mostrar su puesto en los espacios provistos

(Declaración Certificatoria, véase el 49 CFR 390.19).

Pasajeros Privados (Negocios) — un autotransportista privado contratado en la transportación interestatal de pasajeros la cual se provee

para el fomento de una empresa comercial y no está disponible al público en general (e.g., orquestas).

Pasajeros Privados (No de Negocios) — un autotransportista privado involucrado en la transportación interestatal de pasajeros que por

lo demás no cumple con la definición de un autotransportista privado (negocios) (e.g., autobuses de iglesias).

Migratorio — transportación interestatal, incluyendo a un transportista contratista pero no a un transportista común, de 3 o más

trabajadores migrantes hacia o desde su empleo por cualquier vehículo motor que no sea un automóbil de pasajeros o vagoneta.

Correo de EE.UU. — transportación del correo de EE.UU. bajo contrato con el Servicio Postal de EE.UU.

Gobierno Federal — transportación de bienes o pasajeros por una dependencia del gobierno federal de EE.UU.

Gobierno Estatal — transportación de bienes o pasajeros por una dependencia de un gobierno estatal de EE.UU.

Gobierno local — transportación de bienes o pasajeros por un municipio local.

Tribu India — transportación de bienes o pasajeros por el gobierno de una tribu india.
