Figure 3-8 shows the screen that is activated when the "Driver(s)" tab is selected. The statement, "Driver Information (multiple entries permitted)," is followed by 18 fields or checkboxes: "Last Name," "First Name," "Middle," "License Number," "Issued in State," "Date of Birth (month, day, and year)," "Date of Hire ((month, day, and year)," "Owner Operator?," "Co-Driver?," "Experience (in months)," "Endorsements: HM, Tank, or Double/Triple," "Prior Accidents (previous 3 years)," "Died in Accident?," "Died on (month, day, and year)," "Injured in Accident?," and "Suspended?" Buttons for navigating among the records for drivers are located below the last field, and buttons for navigating among the records for vehicles are located below them. Other navigational buttons are located at the bottom of the screen. The "Incident Reports" tabs shown in Figure 3-2 are included at the top of the screen.
