Figure 3-7. Carrier Information

Figure 3-7 shows the screen that is activated when the "Carrier" tab is selected. The field labeled "US DOT Number" appears below the tabs, with a button labeled "Add US DOT Number" to the right. Carrier Information is composed of seven fields: "Name," "Address," "City," "State," "Zip," "Phone," and "Fax." Buttons for navigating among the records for carriers are located below the last field, and other navigational buttons are located at the bottom of the screen. The "Incident Reports" tabs shown in Figure 3-2 are included at the top of the screen.
