Figure 3-2

Figure 3-2 shows the "Incident Reports" screen that is activated when the "Incident Notification" button on the starting screen is pressed. Seven tabs are located at the top of the screen, and navigational buttons are located at the bottom. The tabs are labeled "Incident," "Location," Incident Details," "Agency Response," "Vehicles Involved," "Fatalities/Injuries," and "Notifications." Thirteen fields or checkboxes compose the "Incident" screen. General fields or checkboxes, located under the tabs, include "Incident Key," Date of Incident," "Crash Involved?," Hazardous Material Incident?," "Incident Time (in hours and minutes, AM or PM)," and Information Source." The fields in the middle of the screen pertain to the Division Office and are labeled "Notified on Date (month, day, and year)," "Time (in hours and minutes, AM or PM)," "Response Date (month, day, and year)," and "Response Time (in hours and minutes, AM or PM)." The next two fields pertain to Headquarters Notification and are labeled "Notified on Date (month, day, and year)" and "Time (in hours and minutes, AM or PM)." A "Description" field is provided for the remarks from the HMIS record.
