Figure 3-12 shows the screen that is activated when the "How" tab is selected. The phrase, "How Package(s) failed," is followed by eight checkboxes in two columns. In the left-hand column they are labeled "Punctured," "Cracked," "Burst/Internal Pressure," "Ripped," and "Crushed." In the right-hand column they are labeled "Rubbed/Abraded," "Ruptured," and "Other." A field labeled "Specify" is located below the right-hand column. The "Packaging Information" tabs shown in Figure 3-9 are located at the top of the screen.
