Figure 3-10 shows the screen that is activated when the "Actions" tab is selected. The phrase, "Action contributing to packaging failure," is followed by 18 checkboxes in three columns. In the first column they are labeled "Transport Vehicle Collision," "Transport Vehicle Overturn," "Overloading/Overfilling," "Loose Fitting Valves," "Defective Fitting Valves," "Dropped," and "Struck/Rammed." In the second column they are labeled "Improper Loading," "Improper Blocking," "Corrosion," "Metal Fatigue," "Friction/Rubbing," "Fire/Heat," and "Freezing." In the third column they are labeled "Venting," "Vandalism," "Incompatible Materials," and "Other." A field labeled "Specify" is located below the third column. The "Packaging Information" tabs shown in Figure 3-9 are located at the top of the screen.
