USDOT Number - Application Form
We have provided several ways to fill out the MCS-150 form application for a USDOT number. By following this link, you will be taken to a page where you may select from either downloading and filling out a PDF file that can be faxed or mailed in for processing, or you can fill out the application online. Please note that you will need a valid Visa or Master Card to act as your digital signature if you wish to submit your application on line. The approval of your Visa or Master Card will only be used for digital signature purposes, and there is no charge for filling out the application on line.
